Description:

The Family Advocate is primarily responsible for providing in-home intensive case management services to families receiving treatment. Meets with family in their home to complete the CRS psychosocial assessment for the purpose of identifying the family’s strengths and needs. Develops a case management treatment plan that will address the concerns of both the family and the treatment team. Must meet the standard minimum amount of billable services required for the position.

Qualifications:

Bachelor’s Degree in Social Work or related field preferred.

Must have 3 years of Case Management experience.

Bilingual in English / Spanish a must.

Full time position; with some evenings.
